What Is P21?
P21, usually written P021 in the scientific literature, is a synthetic peptide mimetic designed to reproduce the neurotrophic activity of ciliary neurotrophic factor (CNTF) in a small, druggable molecule. It was derived from the most active region of CNTF by epitope mapping.[1]
The rationale was practical. Whole neurotrophic factor proteins had been tested for Alzheimer’s disease, but those efforts were limited by blood-brain barrier impermeability, short half-life and severe side effects. P021 was developed to overcome those obstacles.[2][1] Its developers describe it as both neurotrophic and neurogenic, meaning it was studied for effects on neuronal support and on the generation of new neurons.[1]
The name can cause confusion. P021 is unrelated to p21, also known as CDKN1A, a cell-cycle regulatory protein. Studies of that protein are not relevant to this compound and are not discussed here.
All published evidence on P021 is preclinical, and most of it comes from the laboratory that discovered it.[2]
Chemical Structure and Molecular Identity
P021 is an acetylated, amidated peptide written Ac-DGGLAG-NH2, consisting of a CNTF-derived tetrapeptide with an adamantylated glycine at its C-terminus. Some papers write it Ac-DGGL(A)G-NH2 to show that the final residue is a modified glycine.[3][4]
The core tetrapeptide comes from the biologically active region of human CNTF. The adamantylated glycine was added to increase blood-brain barrier permeability and to reduce degradation by exopeptidases.[1][5] The N-terminal acetyl and C-terminal amide caps are common strategies for limiting peptide breakdown.
Identity notes
- Names: P21, P021, Peptide 021
- Notation: Ac-DGGLAG-NH2 or Ac-DGGL(A)G-NH2
- Parent protein: ciliary neurotrophic factor (CNTF)
- PubChem: no compound record, so no formula, molecular weight or CAS number is listed here
Because the adamantylated glycine is not a standard amino acid, P021 has no conventional one-letter sequence. Its identity is normally confirmed by mass spectrometry.
Research History and Development
P021 was developed in the laboratory of Khalid Iqbal as part of a program seeking small neurotrophic compounds for Alzheimer’s disease and related disorders. A 2022 review from that group describes it as discovered in their laboratory and summarizes its preclinical studies.[2]
Early work focused on aging. A 2014 study tested chronic oral P021 in aged Fisher rats.[6] The same year, a longer study in 3xTg-AD mice, a triple transgenic Alzheimer’s model, examined whether it could change disease pathology at moderate and severe stages.[3]
Later studies moved treatment earlier in life. Investigators started P021 before overt amyloid or tau pathology,[1] from prenatal day 8 to postnatal day 21,[7] and from birth to postnatal day 120,[4] testing whether early neurotrophic support could prevent Alzheimer-like changes. In 2024, a separate group tested it in models of CDKL5 deficiency disorder, noting that earlier preclinical work had linked P021 to increased BDNF in models of Alzheimer’s disease and Down syndrome.[5]
A 2023 review of peptide- and oligonucleotide-based approaches to tauopathies discussed P021 as a peptide candidate based on preclinical studies.[8]
Mechanisms Studied in Research
The main mechanism studied for P021 is increased expression of brain-derived neurotrophic factor (BDNF), followed by changes in neurogenesis, synaptic proteins and tau-related kinases. These effects were measured in rodent brain tissue and cultured cells.[6][3]
BDNF and GSK3β
In 3xTg-AD mice, P021 increased BDNF expression and decreased glycogen synthase kinase-3β (GSK3β) activity. The authors proposed this pathway as the likely basis for reduced tau hyperphosphorylation.[3] In aged rats, P021 also increased BDNF expression.[6]
Downstream signaling
When given from birth in 3xTg-AD mice, P021 increased pCREB and BDNF expression and activated PLC/PKC, MEK/ERK and PI3K/Akt signaling.[4] After prenatal to early postnatal treatment, it ameliorated postsynaptic markers including PSD-95 levels and CREB activity.[7] In human SH-CDKL5-KO neuronal cells, it corrected alterations in GSK3β signaling.[5]
Amyloid
Its effect on amyloid β was narrower than its effect on tau. In older 3xTg-AD mice, P021 lowered soluble Aβ and produced a trend toward reduced plaque load in the hippocampal CA1 region, consistent with reduced Aβ generation rather than clearance.[3]
Key Findings From Published Studies
In rodent models, P021 has been reported to improve memory, neurogenesis and synaptic markers and to reduce tau pathology, but one model failed to show in-vivo benefit. No human clinical data appear in the studies reviewed here.
Aging and Alzheimer’s disease models
- Aged rats: In 22- to 24-month-old Fisher rats, chronic oral P021 significantly reduced age-related learning and memory decline, countered the neurogenesis deficit, restored cortical and hippocampal synaptic deficits, and lowered hippocampal myoinositol measured by in-vivo magnetic resonance spectroscopy. Myoinositol was elevated in aged compared with young rats.[6]
- 3xTg-AD mice, established disease: Twelve months of treatment starting at 9 to 10 months of age significantly reduced tau hyperphosphorylation and accumulation and rescued deficits in cognition, neurogenesis and synaptic plasticity.[3]
- 3xTg-AD mice, before pathology: Treatment begun at 3 months rescued dendritic and synaptic deficits, boosted neurogenesis and reversed cognitive impairment.[1]
- Prenatal to early postnatal treatment: P021 rescued cognitive deficits at 4 months and, at 22 months, reduced tau pathology, Aβ plaque load and the neuroinflammation marker GFAP.[7]
- Birth to postnatal day 120: P021 prevented cognitive impairment and ameliorated synaptic protein deficits in 4-month-old 3xTg-AD mice.[4]
CDKL5 deficiency disorder
In SH-CDKL5-KO cells, P021 restored deficits in neuronal proliferation, survival and maturation. In Cdkl5 knockout mice, however, chronic treatment did not increase BDNF levels and did not improve neuroanatomical measures, a result the authors described as unexpected.[5] This contrast between cell and animal results is one of the few clearly negative findings in the P021 literature.
Summary of study models
| Model | Evidence type | Reported outcome |
|---|---|---|
| Aged Fisher rats | Animal | Less memory decline; more neurogenesis and BDNF |
| 3xTg-AD mice, established pathology | Animal | Less tau pathology; improved cognition |
| 3xTg-AD mice, early or prenatal start | Animal | Prevented cognitive and synaptic deficits |
| SH-CDKL5-KO human neuronal cells | In vitro | Restored proliferation, survival and maturation |
| Cdkl5 knockout mice | Animal | No BDNF increase; no neuroanatomical improvement |
Because P021 contains a non-standard adamantylated residue, researchers typically confirm identity and purity with HPLC and mass spectrometry certificates of analysis.
Regulatory and Research Status
P021 is not approved by the FDA or any major regulator for any human use. It is an investigational research peptide with no published human trial results.
Its evidence base is limited to cell culture, rats and mice. Most studies come from the group that discovered it, which framed it as a candidate for Alzheimer’s disease and related tauopathies.[2][8] The negative in-vivo result in Cdkl5 knockout mice shows that findings in one model do not automatically carry over to another.[5]
Reviews of the field place P021 among several experimental strategies for tauopathies, alongside antisense oligonucleotides that reduce tau expression. That review noted that one antisense oligonucleotide, BIIB080, had entered clinical development, while P021 was discussed on the basis of preclinical studies.[8] Open questions include its pharmacokinetics in other species, its effects beyond the transgenic models studied, and independent replication of the Alzheimer’s model findings. P021 is supplied for laboratory research only.

Frequently Asked Questions
Is P21 the same as p21 (CDKN1A)?
No. P21 in the research-peptide context means P021, a synthetic peptide mimetic derived from ciliary neurotrophic factor (CNTF). The p21 protein, also called CDKN1A, is an unrelated cell-cycle regulator encoded by a human gene. Literature searches for one often return the other, so researchers should check that a paper describes Ac-DGGLAG-NH2 before relying on it.
What is the structure of P21 (P021)?
P021 is written Ac-DGGLAG-NH2. It consists of an Asp-Gly-Gly-Leu tetrapeptide from the active region of CNTF, followed by an adamantylated glycine, with an acetylated N-terminus and an amidated C-terminus. The adamantyl group was added to improve blood-brain barrier permeability and resistance to exopeptidases. PubChem has no compound record for it.
What does the research on P21 show?
In aged rats and 3xTg-AD transgenic mice, P021 was reported to increase BDNF, boost neurogenesis, restore synaptic markers, reduce tau hyperphosphorylation and improve learning and memory. In Cdkl5 knockout mice, chronic treatment did not raise BDNF or improve neuroanatomical measures. All of this evidence is preclinical, and no human trial results have been published.
Is P21 FDA approved?
No. P021 is not approved by the FDA for any use. It is an investigational research peptide studied in cell culture, rats and mice, and no human clinical trial results appear in the published literature reviewed for this page. Its developers have described it as a candidate for Alzheimer's disease and related tauopathies based on preclinical data.
How is P21 different from CNTF?
Ciliary neurotrophic factor is a full-length protein that does not cross the blood-brain barrier well and has a short half-life. P021 keeps only a short peptide from CNTF's most active region and adds an adamantylated glycine and terminal caps, a design meant to make the molecule stable and brain-penetrant in animal studies.
How is P21 different from Dihexa?
Both are small, modified peptides studied for memory in rodent models, but they come from different parent molecules. P021 is derived from ciliary neurotrophic factor and is studied mainly for raising BDNF. Dihexa is derived from angiotensin IV and is studied mainly for hepatocyte growth factor (HGF)/c-Met signaling.
